BILLING POLICY

 

Insurance:

Free Your Speech is in-network with Blue Cross Blue Shield PPO of Illinois, including BCBS Blue Choice.

If you have Illinois Blue Cross Blue Shield PPO, please email us at fystherapy@protonmail.com or call us with the following information to authorize speech-language services:

​

• The member's name.

• Your child's name and date of birth.

• Your address and phone number.

• A photo of the front and back of your BCBS card (or all the information from your card, including phone numbers).

• A doctor's script for a speech-language evaluation & treatment.

*Please note that authorization of services does not guarantee reimbursement.

Once we have received your information, we will contact BCBS to complete the necessary insurance authorization.  Please note that some BCBS PPO plans do not cover speech-language therapy.  Families are responsible for any fees that BCBS does not cover.  Families are also encouraged to contact Blue Cross Blue Shield to verify their benefits.

We also accept cash payments.

 

Contact FYS Therapy to receive an outline of our out-of-pocket costs. Some insurance plans will reimburse for therapy that was paid out of pocket. It is the client's responsibility to contact their insurance provider to learn their rules and regulations. FYS Therapy will gladly supply a Superbill when necessary.
